How to Get Started

Welcome to Hualapai Senior Services

If you are new to Hualapai Senior Services, this page will guide you through the steps to access available programs and support. Our goal is to make the process simple and help you or your loved one receive services as quickly as possible.

If you are unsure where to begin, you can always contact our office for assistance.

Most Senior Services programs are designed for adults who meet the following general criteria:

  • Must be 55 years of age or older (some programs may vary)
  • Must meet program-specific requirements
  • Must be an enrolled Hualapai Tribal Member for certain services (such as housing)
  • Must demonstrate need based on program guidelines
  • Must be within service area (Hualapai Reservation, Peach Springs, Arizona)

Each program may have additional eligibility requirements listed on its individual page.

Once you believe you may qualify, contact our office to begin the intake process.

You will be asked for basic information such as:

  • Your name
  • Contact information
  • Service needs
  • Basic eligibility details

Staff will help guide you to the appropriate program(s).

Depending on the services you are requesting, you may need to complete one or more forms, such as:

  • Intake/Application Form
  • Transportation Request Form
  • Home Modification Application
  • Caregiver Support Forms
  • Housing Application
  • Consent or Authorization Forms

Forms can be completed with staff assistance if needed.

After forms are submitted:

  • Staff will review your application
  • Eligibility will be determined
  • A needs assessment may be completed (if required)
  • Additional documentation may be requested

For certain services, a home visit or consultation may be scheduled.

If approved:

  • You will be notified by Senior Services staff
  • You will receive information about your approved services
  • You will be given instructions on how to begin receiving services
  • You may be placed on a waitlist if services are limited

Once enrolled, you may begin receiving services such as:

  • Meals and nutrition support
  • Transportation assistance
  • Caregiver support services
  • Home modification assistance (if eligible)
  • Community programs and activities
  • Case management and outreach support

Staff will continue to support you as your needs change over time.
